We are pleased to invite you into our amazing family of clients at our North Scottsdale office. Our mission is deliver incredible patient results within a warm and friendly environment. 

The following documents and forms are being provided to you in advance of your first appointment in order to minimize your in office wait time and to allow you time to review our policies and paperwork for your first appointment.

What we need from you today:

  2. Fill-in contact form with your personal details selecting that you either do or do not agree to our updated policy.
  3. Once we receive your response we will confirm your appointment.
  4. Homework for you - feel free to download the rest of your New Patient Paperwork by clicking the link to the file download below. You can fill it in and bring with you or you can fill in online and attach to an e-mail and return to info@vitalityskinandbody.com.

Your contact details and personal information are kept confidential at all times and are never shared or discussed with people or entities outside of Vitality Company offices and our trained personnel.

    Updated Scheduling & Cancellation Policy Confirmation

​PATIENT APPOINTMENT SCHEDULING & CANCELLATION POLICY

At Vitality Skin & Body we work hard to get our valuable patients in to see our fabulous technicians at their preferred times so we have updated our scheduling policy to ensure that we are getting up to date information to better manage our technician’s availability in the event of changes.

At the time of booking your appointment with us we will obtain a current, valid credit card for your file. In the event you do not show for your appointment or call us to reschedule or cancel your appointment at least 48 hours in advance, your card will be billed a cancellation fee of $75.00.

Your credit card will NOT be charged unless you do not show or cancel your appointment outside of the required 48-hour time-frame Monday – Saturday 9:00 am - 5:00 pm (Sunday is not a business day if you have an appointment on Monday – you would need to inform us by 5:00 pm on Friday of your intent to cancel or reschedule).

In the event of 2 or more cancellations for the same treatment we reserve the right to pre-authorize a non-refundable fee of $75.00 which will be credited towards your selected services after seeing one of our technicians.
 
If you are scheduling an ‘Injectable Procedure’ such as Botox®, Juvederm or Voluma we will need to secure a $100.00 deposit at the time of booking your appointment. The deposit will go towards the costs of your service and in the event of inadequate notice to cancel your appointment or if you do not show we will retain the deposit to cover costs to the practice and technician.

By signing this form you acknowledge and agree to the appointment scheduling policy and understand the cancellation/no-show fees as described above.

This policy has become standard and is explained verbally over the phone to all of our new and existing patients as of 9/27/2016. We cannot confirm new or future appointments without this form being signed or acknowledged online through your form submission.
